We are open to serve you, answer questions or schedule an appointment during the following hours:

Salt Lake City Office: Mon–Fri: 8 am – 5 pm

Moroni Office: Mon–Fri: 8 am – 5 pm

Ephraim Office: Mon–Fri: 8 am – 5 pm

Making an Appointment

Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you may be subject to an additional fee. We see patients by appointment only.

A word about insurance

Insurance Policies

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs.

INSURANCE

Remember that insurance policies vary, and it is important for you to know your insurance benefits before treatment is started. We will review your insurance policy with you to determine the appropriate course of action and answer any questions. Once treatment has started, we will file your claims.

We work with most insurance companies and are preferred providers for the following:

Aetna
BC/BS
Cigna
Delta Dental
Dental Select
Guardian
MetLife

To help us with your insurance filing, please complete the insurance questions on our questionnaire.

Remember that orthodontic insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges. Please remember that it is also your responsibility to inform our office if your insurance company has changed.

ADDITIONAL FINANCIAL OPTIONS

We strive to provide you with affordable, high-quality treatment that fits your budget! Our office accepts cash, check, and MasterCard, Visa and American Express credit cards. In-house, interest-free payment plans are available, and we also offer third-party financing through CareCredit.

We give all patients a login and password which provides access to online payments, receipts, and e-mail and text message appointment reminders.

We will thoroughly explain your options and treatment fee from the beginning so you know what to expect!
